Respected Sir/Madam,
With reference to the above, I beg to inform you that I have been a bonafied customer of Indane Gas for the last 12(Twelve) years. My name was correctly spelled in English in the card when issued by the distributor of Raigarh(C.G). Also name is correctly spelled in Hindi as shown in the card issued by Mahamaya Indane. I also informed you that all the documents which was submitted by me to the distributors of Raigarh(C.G) & Bilaspur(C.G) time to time as demanded having the correct spelling of name (i.e SUJIT MAJI).
But my name shown in Cash Bill/Memo in English is wrongly spelt. Therefore, I request you to kindly correct the spelling as follows:
WRONG SPELLING : SURJIT MAJHI
CORRECT SPELLING: SUJIT MAJI
