1. That I was working in M/s Rangammal Memorial Rehabilitation Society, Tiruvannamalai (Tamil Nadu) from 01.02.2009 to 27.11.2013.
2. My EPF No. is TB / VLR / 0036250 / 000 / 0000749. My case is being dealt with by the EPF Office, Vellore (Tamil Nadu).
3. That I wanted to get my EPF amount Refunded to me. But my employer was not cooperating me in this regard.
4. Therefore, I sent my Form-19 and Form-10-C directly to the EPFO Office, Vellore on 15.04.2014 duly attested by a Gazetted Officer.
5. That on equiry from the Website, I have come to know that my case has been rejected by the EPF Office and in this connection, a letter has been dispatched at my address.
6. That a period of about four months has passed, but no letter has been received at my address given in my Form -19 and Form -10-C.
7. Kindly direct the EPF Office to settle my Refund case at the earliest.
