Dear Sir, Claim # 91740578 00/91740578 01/91740578 02 Policy No : 13924670 Proposer Name : KHAN NISHAT RASHID Patient Name : FARIDA R KHAN Insurance Company: Care Health Insurance Limited (Formerly known as Religare Health Insurance Company Limited) My Mother FARIDA R KHAN was suffering from COVID-19 and was Hospitalized on 06th May 2021 and she died on 11th May 2021 for the said reason, I have applied for the claim but unfortunately the Insurance Company rejected the claim by mentioning NON DISCLOSURE of existing diseases, I am not satisfied with their reason of rejection and therefore I am taking your route to get justice in my case. In my support I would like to mention few points which Insurance company has totally ignored while making their decision: First of all, I have not hidden the any known facts before issuance of policy, As a layman in medical sense, I have declared all the known diseases which my Mother was suffering from, So there is no question of any Non-disclosure from my end. Secondly, My mother was infected with COVID-19 and there is no correlation between the pandemic disease and any of her pre-existing diseases, Please find in attachment RTPCR report & her Death Certificate- which clearly & loudly mentions about the reason of admission & death of my mother. Thirdly, I have been paying the policy premiums regularly since March 2019 & my policy was Active when she got hospitalized due to COVID-19 in May 2021. Fourth important fact is that I have not claimed anything since the inception of the policy (i.e. March 2019). So, In that case, I request you to please intervene in my case & provide me justice, as a Service Class employee in Private firm I have only one source of income & I have spend almost all my savings behind my mother’s treatment and I am a regular premium payer, I request you once again to look into my case & help me, Thanking you, Yours Truly, Nishat Khan
